In a Standalone (non-RAC) environment, database creation fails:

ORA-00200: control file could not be created
ORA-00202: control file: '+LOG1'
ORA-17502: ksfdcre:4 Failed to create file +LOG1
ORA-15001: diskgroup "LOG1" does not exist or is not mounted
ORA-15077: could not locate ASM instance serving a required diskgroup
ORA-29701: unable to connect to Cluster Manager

Also create diskgroup errors out with ORA-15018 but gives no additional information.

IMPORTANT:  This article only applies to Oracle pre-11.2 Release, on UNIX (not Windows) and Standalone (non-RAC) environments .

For both cases/errors, the following checks were done:

* ocssd.bin daemon is up/running: ps -ef|grep d.bin| grep -v grep
* init.cssd entry in /etc/inittab
* ASM instance is up/running with ASM diskgroups mounted
* Also there are other databases that store their data in ASM and these are up/running
* ASM disks are visible from ASM 's v$asm_disk for the create diskgroup statement that fails.
