RAC Instance Fails to Start with Error 'LMON: terminating the instance due to error 481' as Parameter cluster_database was Removed in pfile/spfile (Doc ID 1514348.1)

Symptoms

On a multi-node RAC cluster, after rebooting one of the nodes (e.g. for routine node maintenance), ASM instance starts up fine but the database instance on the rebooted node  cannot rejoin the cluster and is terminated immediately after start up. All other instances are up & running (no restart was done on the other nodes).

The alert log for the failing instance shows:
