Symptoms
Oracle Grid Infrastructure sometimes fails to start on Linux as init.ohasd process is not being started by "init" process.
Output of strace of ohasd.bin shows lots of:
open("/var/tmp/.oracle/npohasd", O_WRONLY|O_NONBLOCK) = -1 ENXIO (No such device or address)
