CTX_DOC.FILTER Hangs when Indexing got DRG-11207: User Filter Command Exited With Status 721035 (Doc ID 1901663.1)

Symptoms

When reading the clob using ctx_doc.filter the sessions hangs.
We see this happening after:
a) having the DB upgraded from 11.2.0.3 to 11.2.0.4
b) having ctxsys.drvdml.auto_sync_index returned with status DRG-11207: user filter command exited with status 721035

We have sort of 40 records (on 23 mln) which had this status on the sync_index job.
These 40 cases, the clobs are inserted during a period of the last 4 years. It seems like it only happens with Word 97-2003 formatted clobs.

When on 11.2.0.3 ctx_doc.filter has no issues.
