Symptoms
With parameter os_authent_prefix = 'OPS$' and a user created as "OPS$USERNAME" identifed by password, you can no longer logon locally.
Changes
Upgrade to version 12.1.0.2 or higher. This includes the changes made as part of : Bug 16072125 : DEFAULT VALUE OF OS_AUTHENT_PREFIX PARAMETER IS SET TO OPS$
