Asmcmd Afd_configure Fails With ASMCMD-9524: AFD Configuration Failed (Doc ID 1940863.1)

Symptoms

 In a 12.1.0.2.1 Oracle restart environment afd_configure command to implement the ASM Filter Driver returns the error:
ASMCMD-9524: AFD configuration failed 'ERROR: OHASD start failed'

The devices are able to be labelled ok during the session but once the host is rebooted it seems like the driver isn't loading/scanning on boot
A manual setting of the afd_dsset and then scanning relocates the devices and they're present again but rebooting loses config once more.

Restart "has" and then define and migrate to AFD disks and it seems to work but as soon as we reboot the host it seems like the config isn’t persistent. We can startup the system, run afd_dsset and afd_scan manually and the disks reappear.

ASMCMD> afd_label RECO1 /dev/asm-reco01 --migrate
ORA-15227: could not perform label set/clear operation
ORA-15225: label name 'RECO01' is already in use
ORA-15224: disk "/dev/asm-reco01" is already provisioned for ASM (DBD ERROR: OCIStmtExecute)
ASMCMD-9513: ASM disk label set operation failed.

ASMCMD> afd_scan
ASMCMD> afd_lsdsk
There are no labelled devices.
