Symptoms

1) Lots of hs_core* files are being generated in the $ORACLE_HOME/rdbms/log directory on UNIX or the %ORACLE_HOME%\rdbms\trace directory on Windows.

2) The database version is 12.1.0.x or 11.2.0.4.

Changes

The database has either been upgraded to 12.1.0.x or 11.2.0.4, or it is a new installation of 12.1.0.x or 11.2.0.4.
