Symptoms
1. The below error is logged in the alert log, which may or may not be followed by instance crash:
ORA-07445: exception encountered: core dump [kcbm_sim_one()+1288] [SIGSEGV] [ADDR:0xC109CCD18] [PC:0x104D4FF68] [Address not mapped to object] []
2. According to the error incident trace file :
The failing statement can be an INSERT statement.
