Database can't started due to quorum disks are not mounted (Doc ID 2247653.1)

Symptoms

This is a RAC enabled database running on Exadata rack.

Database failed to be started, the following error reported in GI alert log:

PRCR-1013 : Failed to start resource ora.XXX.db
PRCR-1064 : Failed to start resource ora.XXX.db on node XXXX02
CRS-5017: The resource action "ora.XXX.db start" encountered the following error:
ORA-00205: error in identifying control file, check alert log for more info
. For details refer to "(:CLSN00107:)" in "/u01/app/oracle/diag/crs/XXXXXX02/crs/trace/crsd_oraagent_oracle.trc".

 

Database alert log shows the following errors:

ORA-15001: diskgroup "DATA" does not exist or is not mounted 
ORA-15040: diskgroup is incomplete
ORA-15040: diskgroup is incomplete
ORA-205 signalled during: ALTER DATABASE MOUNT /* db agent *//* {1:35991:10107} */...

 Although the alert log message indicate the diskgroup DATA is not mounted, the fact is it has been mounted.
