Symptoms

Upgrading Grid Infrastructure from 12.1.0.2 to 12.2.0.1, rootupgrade.sh succeeds on node1 but fails on node2:

ORA-03113: end-of-file on communication channel
Process ID: 0

CRS-2674: Start of 'ora.asm' on 'node2' failed
