Symptoms

Following successful Grid Infrastructure installation / configuration in a 2-node RAC on Windows cluster, the install node server is crashing from an Oracle Clusterware node eviction, specifically with Bugcheck 0x0000fffe, which tells us that the OraFence driver has not become scheduled on the DPC (Deferred Procedure Call) queue within its allotted timeout.

The following is reported in the Windows System Event Viewer log:
