Symptoms

SQL> connect sys/******@<connect string> as sysdba
ERROR:
ORA-01017: invalid username/password; logon denied
ORA-17503: ksfdopn:2 Failed to open file +DATA/FSYS/orapwfsys
ORA-01017: invalid username/password; logon denied

Changes

 The instance and the listener were restarted.
