Symptoms

Unable to create the Oracle Database Appliance after a Bare Metal Restore to 12.2.1.x

1.  Error thrown when applying the Server Patch for ODACLI/DCS Stack:   

 
In 12.2 on ODA the ASM header information from your previous installations may not be cleanly removed from any previous installation.
It is assumed the Bare Metal Restore) BMR will clear the previous ASM disks on the disk tray- but it doesn't
