Database (12.1.0.2) creation using DBCA fails with errors PRCD-1095 and PRKC-1137

Symptoms
+ Database is created on Exadata platform.
+ DBCA GUI reports below errors on screen :
PRCG-1036 : Internal error
PRCD-1095 : Failed to retrieve databases for version 11.1 and earlier
PRKC-1137 : Unable to find version object with string value 19.0.0.0.0
+ Database creation eventually fails with below error stack:
ORA-01501: CREATE DATABASE failed
ORA-00200: control file could not be created
ORA-00202: control file: '+DATA'
ORA-17502: ksfdcre:4 Failed to create file +DATA
ORA-15001: diskgroup "DATA" does not exist or is not mounted
ORA-15040: diskgroup is incomplete
+ There are already two other databases running at 19c and 12.1 (from another Oracle home) registered in OCR.
+ Issue seem closely related to bug 20007009. But, customer already applied OCW PSU 12.1.0.2.200414 on Oracle home.
Changes
+ Customer installed 12.1.0.2 database binaries under new home and then attempted to create Database by invoking DBCA from newly created 12.1.0.2 database home.
+ Grid Infrastructure (GI) and ASM running on 19c.
