Symptoms

DB instance terminated with below message in SMON trace file :

ORA-00603: ORACLE server session terminated by fatal error
ORA-24557: error 601 encountered while handling error 1; exiting server process
ORA-00601: cleanup lock conflict
ORA-00001: unique constraint (SYS.I_MON_MODS$_OBJ) violated
