Upgrade to BDA 5.1 UpdateBaseImage Step 2 Continuously Slates All Nodes for Reboot

Symptoms
Note: In the examples below, user details, company name, email, hostnames, etc. represent a fictitious example. Any similarity to actual persons, or entities, living or dead, is purely coincidental and not intended in any manner.
Upgrade to BDA 5.1 UpdateBaseImage, Step 2, slates all nodes for reboot as expected like:
INFO: Time spent in step 2 UpdateBaseImage is <NUMBER_SECONDS> seconds.
INFO: Skipping password-less ssh check
WARNING: The OS kernel was updated on some nodes - so those nodes need to be rebooted
INFO: Nodes to be rebooted: <HOSTNAME1>,<HOSTNAME2>,<HOSTNAME3>,<HOSTNAME4>
Proceed with reboot? [yes/no]: yes
INFO: Skipping password-less ssh check
WARNING: The OS kernel was updated on some nodes - so those nodes need to be rebooted
INFO: Nodes to be rebooted: <HOSTNAME1>,<HOSTNAME2>,<HOSTNAME3>,<HOSTNAME4>
Proceed with reboot? [yes/no]: yes
But no matter how many times the nodes go down for successful reboot, the Mammoth upgrade slates them for reboot again.
Other symptoms:
1. The file /root/BDA_REBOOT_REQUIRED is regenerated over and over for each host no matter how many successful reboots there are.
