Instance Fails To Start With ORA-7445 [PC:0xEE00] And ORA-3113 (Doc ID 2983883.1)

Symptoms

On AIX 7.2 19c 2-node RAC, database instance on node1 fails to start with the following errors.

The truss log during instance startup showed it stopped at kwaitpid after kfork.
