Symptoms
The following error is reported in a trace file for one of the Queue Monitor worker processes on one node in a RAC cluster
kswsreasvc : unexpected error from fetch : rpi_rc = 1403
with the additional error
ORA-00600: internal error code, arguments: [1:kwqvss], [2], [APPLSYS.WF_CONTROL]
which has the following stack trace
kwqvss -> kwqvss_cbk -> kwqvqsi -> kwqasown -> kwqvqsi -> ktcccdel
This error can be reported repeatedly causing the instance to crash.
