Symptoms

Trying to create a new database with DBCA fails with the following errors.

Process PMON died, see its trace file
USER (ospid: 4639): terminating the instance due to error 443
Instance terminated by USER, pid = 4639

Changes

New 11.2 installation and first database creation.
