Symptoms
On OTM Version, 6.0.3,
When creating an accessorial cost, only the cost type of "external" has the fields "Minimum Charge" and "Maximum Charge" available.
EXPECTED BEHAVIOR

The two fields should be available for all cost types.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1- Create an accessorial cost and select any cost type except "External" > Notice that the "Minimum Charge" and "Maximum Charge" fields are not available under those cost types.
