Unable to Save Accessorial Cost When Editing Service Provider and Cost.

Symptoms
When using firefox along with the "Edit Service Provider and Cost" action, accessorials are unable to be saved. When clicking on the "Save" button.
STEPS
-----------------------
1. Log into OTM
2. Search for a shipment
3. Select the shipment and go to Actions --> Change Service Provider --> Edit Service Provider and Cost
4. Type in the following:
Service Provider ID: XXXX (any service provider)
Cost: 500 USD
Cost Type: Accessorial
Shipment Cost: 230 USD
Accessorial Code: FUE (Any code available)
5. Click Save
6. Note nothing happens and the record is not saved.
