R12: "FRM-92050: FAILED TO CONNECT TO SERVER: /FORMS/LSERVLET" Error Message When Launching Forms (Doc ID 1070263.1)

Symptoms

Users are getting FRM-92050 error message while launching forms. This error only occurs in Servlets mode.

Users can log directly into forms using Socket mode as documented in Note 560947.1 - "How To Start R12 In Forms Socket Mode For Testing?"

Log file shows:

 [error] [client xxx.xxx.xxx] [ecid: xxxxxxx.xx.xxx.xxx.xx:x:xx,x] Directory index forbidden by rule:  $COMMON_TOP/java/classes/

formsstd.err
 Error: <connector name="OracleASjms" path="OracleASjms.rar" /> will not be bootstrapped since corresponding module declaration was not found in application.xml.

oafmstd.err
Error: <connector name="AppsSOAAdapter" path="AppsSOAAdapter.rar" /> will not be bootstrapped since corresponding module declaration was not found in application.xml.
Error: <connector name="OracleASjms" path="OracleASjms.rar" /> will not be bootstrapped since corresponding module declaration was not found in application.xml.
 java.sql.SQLException: ORA-20001: The Applications user, ASADMIN is not authorized to access the database.
