BI Publisher (XML Publisher) Reports Failing With APP-FND-01564 ORA-3113 and ORA-03114 Errors (Doc ID 1093949.1)

Symptoms

On 11.5.10.2, when attempting to submit XML/BI Publisher reports the following error occurs:

APP-FND-01564: ORACLE error 3113 in publish
Cause: publish failed due to ORA-03113: end-of-file on communication channel.
The SQL statement being executed at the time of the error was: &SQLSTMT and was executed from the file &ERRFILE.

APP-FND-01564: ORACLE error 1041 in afpprq
Cause: afpprq failed due to ORA-01041: internal error. hostdef extension doesn't exist.
The SQL statement being executed at the time of the error was: &SQLSTMT and was executed from the file &ERRFILE.
ORA-03114: not connected to ORACLE
Routine AFPPRD has encountered a fatal error. ORA-03114: not connected to ORACLE

ORACLE error 3114 in AFPRSR-Resubmit_Time
Cause: AFPRSR-Resubmit_Time failed due to ORA-03114: not connected to ORACLE

The issue can be reproduced at will with the following steps:
Submit an XML/BI Publisher report.

The issue has the following business impact:
All our BI publisher concurrent reports end with errors. (The Employee W2 PDF report is the only one we tested that works).
