When a Transmission Fails, There is No Way of Knowing Without Reviewing the Logs

Find that if the transmission fails to create, the only way to know what happened is to review the log as we would not get a transmission in ERROR status.
Expect to that the user would be alerted that a transmission failed to create rather than have to review the logs.
