E-Business Suite Forms Users Receive Error 'FRM-92101 oracle.forms.net.ConnectionException' When Launching Forms Responsibility and Function Sessions (Doc ID 1258305.1)

Symptoms

When a small number of users attempt to start any forms based application, the following error occurs:

ERROR
FRM-92101 A failure has occurred on the server.
You will need to reestablish your session.

oracle.forms.net.ConnectionException: Forms session <2> aborted:
unable to communicate with runtime process.

at oracle.forms.net.HTTPNStream.doFlush(Unknown Source)
at oracle.forms.net.HTTPNStream.flush(Unknown Source)

at oracle.forms.net.StreamMessageWriter.run(Unknown Source)

Also GL Journal Uploads fail with the following error

"oracle.apps.bne.exception.BneInvalidException"

The issue can be reproduced at will with the following steps:

1:- Log into the application
2:- Choose a forms based responsibility and function.
3:- Forms fails to launch
