Journal Import - GLLEZL Splits Journals By Accounting Date Causing EU02

Goal
Even after following Note 749119.1 R12 Creating Separate Journals Based On Accounting Date for External Journal Import we still get EU02 when importing certain Journals.
We have applied one of the following patches:
12.0.x Patch 8970530:R12.GL.A
12.1.x Patch 9029792:R12.GL.B
and checked the profile 'Journal Import: Separate Journals by Accounting Date' is not set to Yes but import still splits a 3 line journal into two journals which are unbalanced.
The data that was failing EU02 and was checked for the following:
a. different value populated in reference4 to other rows. This reference is used in creating the header field.
b. period
c.currency rate,
d. currency,
e. conversion date
None of these factors were the cause.
