Symptoms
On : 12.1.1 version, Concurrent Processing, SRS
When attempting to submit any concurrent requests from any responsibility, getting the following errors:
"FRM-40735 ON-INSERT Trigger raised unhandled exception" followed by "ORA-01403 , No data found".
This started happening after applying <Patch:9813947> and <Patch:9883001>.

The issue can be reproduced at will with the following steps:
Log into apps - go to any responsibility - submit a concurrent request.
