System Alerts In OAM Not Sending Email Notifications To User

Symptoms
Setup a notification for system alerts in OAM(OAM->Sitemap->Monitroing->System Alerts->Notifications Setup for a particular user, application, component(Concurrent program), for ALL category, ALL Severity. Expectation is to receive a email notification in case of an error, critical, warning to the user email address associated with user name in the user screen. Workflow Notification Mailer is up and running and user can receive a TEST Email notification successfully. Oracle Alert has to place an XML representation of the alert message in agent WF_NOTIFICATION_OUT but we don't see a record in case of concurrent request failure. The User is able to see the alerts in the alert screen within OAM. Do not see a record created in WF_NOTIFICATIONS for message type OAM_BE when the alert is showing in the OAM>System Alerts screen.
