Symptoms
On Applications 12.1.2 :
When attempting to run the standard API GMF_LOTCOSTADJUSTMENT_PUB.create_lotcost_adjustment , the following error occurs.

Warning : Only detail records will be inserted as header record already exists..
Warning : Record keys are ignored as ID is supplied..
This record already exists..
Error : Lot Cost Adjustment Record Legal Entity Id : xxxxx Cost Type Id : xxxx Cost Type : xxxxx Organization Id : xxx Organization Code : CRE Item Id : xxxxxx Item No : xxxxxxxxx Lot Number : xxxxxxx Adjustment Date : xx-xxx-xx failed..

Only the fist record from input is loaded.
