Symptoms

Sales Coach does not save user entered values

1)  Sales Dashboard/Opportunity/Sales Cycle/4 stages
2)  Sales Coach: Consultative/Qualifying/brief intro had 'What is driving business' has 1... more questions with 1's.
3)  Consultative/Solution Build - no answers checked
4)  Sales Coach: Proposing - no answers checked
5)  Sales Coach: Contracting - no answers checked 
Setup Sales Coach with 4 questions (stage), set up in one instance, this was cloned to another test instance.  Each instance has a different question that does not save.
