On : 12.1.3 version, BasicDet,Address,Phone,Contact

When attempting to submit an updated Personal Information record, with an attachment, the following error occurs.

The changes were not applied because User-Defined Exception

Steps to Reproduce:
The issue can be reproduced at will with the following steps:

1) From the Main SSHR Responsibility, navigate to Personal Information.
2) Update Dependents and Beneficiaries
3) Add Attachment as a File
4) Save the attachments, and submit.
5) On the confirmation page, error is encountered.
