Customer Item Cross References Will Not ALWAYS Work With Item Relationships

On : 11.5.10 version, Item Usage
When attempting to use item cross reference and item relationships in an order,
the following error occurs.

FRM-40350: query caused no records to be retrieved
STEPS

The issue can be reproduced at will with the following steps:
Per customer:
We have customer item cross reference as well as item relationships set up in Oracle.
However when we add the customer item xref on line 1 on a sales order line, the system shows the item relationship pop up which allows us to select the superseded item.
When I add the same xref at line 2, the system does not show the item relationship screen to choose the superseded item.
BUSINESS IMPACT

Due to this issue, users cannot choose the superseded item.
