XML Publisher Reports Aren't Submitted From The Forms, But Are Submitted Normally From The Web Interface

Symptoms
Just upgraded from EBS 11i to EBS 12.1.3 and noticed that XML publisher Reports can't be submitted from the forms, but can be submitted normally from the web interface.
When one tries to submitt an XML publisher request from the Core Application forms screen (report with XML or pdf output), one is getting the following errors and the request is not submitted:

FRM-40735: ON-INSERT trigger raised unhandled exception ORA-04062
Although, when running the same request from system administration (web interface), the request is submitted, completes normal, and produces the PDF output successfully.
