Insufficient Privileges At EBS Login Using 11.1.2 Access Manager With Access Gate (Doc ID 1526439.1)

Symptoms

On Oracle E-Business Suite (EBS) 12.1.3: Advanced Configurations

Actual Behavior
After configuring EBS 12.1.3 , Oracle Access Manager 11.1.2 with E business Suite Access Gate 1.2 Single Sign On is work fine as expected. Then in order to have high availability Load Balancer was introduce to single sign on.  Since then users trying to log into EBS get redirect to Oracle Access Manager login page after entering username password page just hangs . HTTP headers and log files shows below error:

You have insufficient privileges for the current operation. HTTP/1.1" 302 301

 
Expected Behavior
With Oracle Application Manager (OAM) load balancing authentication should work.
 
The issue can be reproduced at will with the following steps:
1. Access E business suite URL http://<server name>.<domain name>:<port>
