TeleService - Serial Number: LOV is Empty or Entering Number Produces FRM-40212 Error

In Service Request form:
The Serial Number LOV is empty or users get an error when trying to add a given Serial Number.
FRM-40212 INVALID VALUE FOR FIELD CURRENT_SERIAL_NUMBER
(R) Customer Support
(N) Service Requests > Create Service Requests
- Enter Customer name: CUSTOMER 1 (Account # 12345)
- Enter Item: MYITEM1
- Hit the Tab key
=> Form shows an error at the bottom:
FRM-40212: Invalid value for field CURRENT_SERIAL_NUMBER
What is Working:
LOV may retrieve other Serial Numbers.
