Symptoms

When attempting to complete a work order get the following error

ERROR
-----------------------
EAM_ENTER_FAILURE_INFO


STEPS
-----------------------
1. Responsibility: Maintenance super user
2. Go to work orders and query up a WO to complete
3. Enter the relevant details and save.
4. The above error pops up and we are unable to complete the WO.
This despite the fact that no failure entry is required
