Work Order Has Changed Since You Started Working On It. Please Re-query To See Changes (Doc ID 1557077.1)

Symptoms

On : 12.1.3 version, ENV

When attempting to add resources to express work order, the following error occurs.
The issue ONLY happens for express work orders. Routine work orders do not exhibit the issue.

ERROR
-----------------------
First Save button was pressed, the error occurs: "The work order has changed since you started working on it. Please re-query to see the changes"
If Save is pressed again, the following error occurs:
  1. Resource with Resource Sequence Number 10 already exists.
  2. This record with Inventory Item Id &INV_ITEM_ID and Wip Entity Id 39004 was not processed because a sever occurred in the resource while checking if it exists. (RES_SEQ_NUMBER=10).
  3. Resource with Resource Sequence Number 20 already exists.
  4. This record with Inventory Item Id &INV_ITEM_ID and Wip Entity Id 39004 was not processed because a sever occurred in the resource while checking if it exists. (RES_SEQ_NUMBER=20).


STEPS

The issue can be reproduced at will with the following steps:
1) Responsibility: Maintenance User
2) Click on Work Orders tab
3) Click on Express Work Order subtab
4) Express Work Order was created and completed successfully
5) Two resources were added
6) Save button was pressed and error occurs as: "The work order has changed since you started working on it. Please re-query to see the changes"
7) If Save is pressed again, the following error occurs:
    a. Resource with Resource Sequence Number 10 already exists.
    b. This record with Inventory Item Id &INV_ITEM_ID and Wip Entity Id 39004 was not processed because a sever occurred in the resource while checking if it exists. (RES_SEQ_NUMBER=10).
    c.  Resource with Resource Sequence Number 20 already exists.
    d. This record with Inventory Item Id &INV_ITEM_ID and Wip Entity Id 39004 was not processed because a sever occurred in the resource while checking if it exists. (RES_SEQ_NUMBER=20).
