Symptoms

The email notifications triggered by QA Actions are being sent twice.
Expecting only 1 email notification sent for each of the recipients.

The issue can be reproduced at will with the following steps:
1. Navigation: Quality > Results > Enter > Enter Quality Results
2. Query the plan having email action
3. Enter results and save.
4. Email notification is sent twice (the Quality Plan Action is fired twice)
