AME: Action History Is Incorrectly Updated

Symptoms
On : 12.1.3 version:
The Action History is not correctly updated in certain scenarios when using the Approval Management Engine (AME):
1. The Action History is shown as pending until the requisition is approved by the last approver. A For Your Information (FYI) notification that is sent after all the approval, shows all the approvers status as approved and the comments given by the approvers are not updated in the action history.
2. When an approver approves a requisition using GUEST access, the Action History is not populated with the action "Approved".
3. The Action History is not correctly updated when Delegate mode is used, in the following cases:
RICO (Requester Initiated Change Order)
Multiple Delegations
Request for More Information
4. The Action History in Delegation mode is not getting updated when there are multiple delegations and when the delegated notification times out.
