Setting Up Self Registration in iReceivables and Receive Error - Oracle.jbo.DMLException: JBO-26041: Failed To Post Data to Database During "Rollback to Savepoint" (Doc ID 1621601.1)

Symptoms

On : 12.1.3, SEP 2013 RPC

Attempting to implement iReceivables. 
In the final step of the Self Registration process, you receive the following error:

oracle.jbo.DMLException: JBO-26041: Failed to post data to database during "Rollback to Savepoint": SQL Statement "null".;

Steps to Reproduce:
1. Access the Self Service Portal Page

2. Customer Registration
Click the ‘Register Now’ button
Brings You to the ‘Select Organization (Account)’ page
   i. This page 1 of 6 pages in the registration page flow
Enter the following:
   i. Customer Account Number
  ii. Email Address,
 iii. Confirm email address by re-entering the email address
Click the ‘Next’ button

3. Validate Customer Access (page 2 of 6)
  a. Answer the Customer Challenge Question, and click ‘Next’ button
   i. Temporary Challenge Question and Answer:
      1. Q: We Are
      2. A: Penn State

4. Select the (Bill To) Location (page 3 of 6)
   a. Review the Available Locations
   b. Select the Location to Add by clicking the corresponding radio button
   c. Then, click the ‘Next’ button

5. Verify Access (page 4 of 6)
   a. Answer the 2nd Challenge Question, and click the ‘Next’ button

6. Enter User Information (page 5 of 6)
   a. Mandatory Fields include:
    i. First Name
   ii. Last Name
  iii. Password
  iv. Re-Confirmed Password

7. Review Access Request (page 6 of 6)
  a. Confirm the Information
  b. Click the Terms and Conditions checkbox
  c. Click ‘Submit’ button
