E-Business Centre Form Crashes On Opening With FRM-92101 Error

Symptoms

On : 12.1.3 version, Core

We are trying to access the EBusiness Center form through the 'Telesales Agent' responsibility. However every time we try to open the Form, the Form errors with ,
the following error :

ERROR
-----------------------
FRM-92101 Forms Server failure error:FRM-92101: There was a failure in the Forms Server during Startup.
This could happen due to invalid configuration. Please look into the web server log file for details. As a result, we are unable to access the E-Business Center form.


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. 'Telesales Agent' responsibility
2. Click on ebusiness center
3. Error appears
