SR Tasks: Error 'FRM-41826 Cannot Replace Group; Columns do not Match LOV' When Mapping New Task Types

Symptoms
The Task Type LOV in the Create Service Request form is empty.
When trying to map the Task Type to the SR, there is an error.
Steps:
(R) Service
(N) Service Requests > Create Service Request
* SCENARIO 1:
- Tasks Tab
- Open the Task Type LOV
=> It is empty.
- To add a Task Type against the SR, go to Task Types > Create a new Task Type > Mappings.
- Map the new Task Type to the required Source (i.e. Service)
- When this is attempted, the error pops up:
=> FRM-41826 Cannot replace group: columns dont match LOV
* SCENARIO 2:
When trying to close the SR, another error occurs:
FRM-40735: CLOSE_WINDOW trigger raised unhandled exception ORA-06508.
