Duplicate Contact is Created When Using Tools > Dunning / Collections Contact (Doc ID 1678090.1)

Symptoms

When you Select a Contact Last Name from the List of Values (LOV), in the Collections Agent UI, and then save it as a Collections or Dunning Contact, it replicates that contact on the customer for each time.

Steps to Reproduce:
Responsibility:  Receivables
Navigation:  Customers > Customers
Create Contact on a Customer Account

Responsibility:  Advanced Collections
Navigation:  Collections Agent > Collections
Query the Customer/Organization, select the Contact from the LOV
Go To:  Actions > Collections Contact
Go To:  Actions > Dunning Contact

When you go back into the Customer Standard form, the contact is now listed three times on the Account, with only the phone number>  Where is the fax number?
