Symptoms

Requisition Edit during Request for Information results in Error Message

When the Requisition is edited during "Request for Information" stage, the response cannot be submitted due to following error message:

ERROR
ORA-01403: no data found ORA-01403: no data found ORA-01403: no data found ORA-06512 at "APPS.WF_NOTIFICATION", line 7108 ORA-06512: at line 1


STEPS TO REPRODUCE
The issue can be reproduced at will with the following steps:
1. Create and approve a requisition
2. Submit for approval
3. Approver edits the requisition and submits but it errors with above error
