R12.1.3: PA_Tasks Corruption Whereby Task_id = Top_task_id and Parent_task_id is now Null (Doc ID 1681385.1)

Symptoms

A significant number of projects have the task_id = top_task_id, and the parent_task_id is null but the WBS_Level is not 1. This causing issues with the Forms not displaying tasks correctly, and reports do not have the task/parent/top task relationship correctly. The Workplan and Financial tasks forms show the data correctly.
