Symptoms
On : 12.1.3 version, Request Processing
ORA-01403 error appears when submitting a concurrent request.
FNDRSRUN. fmb form is available but no form menus are available for update.
Issue is not reproducible on all environments.

The issue can be reproduced at will with the following steps:
1. Navigate to Sysadmin Responsibility - Concurrent Request - run.
2. Select the Active Users.
3. See error ORA-01403 in window for new request.

Applied Patch 17839156 R12.PJ_PF.B.delta.6: Oracle Projects Release Update Pack 6 for 12.1 (R12.PJ_PF.B.delta.6).
Patch contains FNDRSRUN.fmb version 120.44.12010000.74 +.
There are several versions of 120.44.12010000.74 +. So ensure header version matches for environment that works and one that does not.
