FNDCPASS Fails With APP-FND-02704: Unable to Alter User APPS to Change Password (Doc ID 1930695.1)

Symptoms

When attempting to change the APPLSYS password,
the following error occurs:

FNDCPASS apps/xxx 0 Y system/yyy SYSTEM APPLSYS zzz
APP-FND-02704: Unable to alter user APPS to change password.


Notes:

- adgrants can be run successfully following Note 1585578.1 - Interoperability Notes Oracle EBS R12 with Oracle Database 11gR2 (11.2.0.3)
 ==> Step 18 Run adgrants.sql

- adadmin task "Re-create grants and synonyms for APPS schema" is also failing with the following error:

alter user CTXSYS quota unlimited on CTXD

ERROR at line 1:
ORA-01031: insufficient privileges

Changes

Environment has been upgraded recently to R12.1.3 / 11GR2

Database Vault has been implemented from Note 1091083.1 - Integrating Oracle E-Business Suite Release 12 with Oracle Database Vault 11gR2
