Symptoms

After upgrading to 12.1.3, when the user enters in the field 'Upon Completion' > Notify when submitting the request, email notification is not being sent intermittently.

Navigation

Submit Request > Upon Completion > Options > Notify

LOGFILE

For the Non-Working case , Request does not have notification id assigned, and following is found in the log: "Sending notification to XYZ.".
