Symptoms
Encountering an intermittent ORA-20001, APP-FND-01971 failure on adding responsibility to user.
This can be produced with the following steps:
1. Have a few errors on adding responsibility to a user.
2. The addition of a responsibility failed to complete with the following error:
"Oracle error-2001: ORA-20001: APP-FND-01971: Error in FND_USER_RESP_GROUPS_API.Insert_Assignment: Cannot insert this row because there is already a direct assignment of the user <username> to the role FND_RESP|SYSADMIN|SYSTEM_ADMINISTRATOR|STANDARD in the Workflow User/Role tables. has been detected in FND_USER_RESP_GROUPS_API.INSERT_ASSIGNMENT."
