TeleService Error: FRM-40735 WHEN-CREATE-RECORD ORA-01403 When Opening Service Request Form (Doc ID 2018421.1)

Symptoms

On 12.2.4 - Newly upgraded 12.2.4 instance:

When trying to create a new service request, receive FRM-40735:  WHEN-CREATE-RECORD trigger raised unhandled exception ORA-01403.

The error occurs just trying to open the 'Create Service Request' screen.
The SR form does not open, therefore users are unable to enter any data.
